At the Geriatric Department, we work together to provide our frail elderly patients with safe and integrated care. As a Discharge Coordinator, you will become an important part of our dedicated team, where collaboration, participation, and patient needs are in focus. As our current colleague is retiring after many years in the role, we are now seeking you to take over an important assignment with great significance for patients, families, and healthcare collaboration. Job Responsibilities At Geriatrics, we work every day to create security and dignity for our frail elderly. As a Discharge Coordinator with us, you have a central and meaningful role in the patient's care journey. You are responsible for planning and coordinating discharge from inpatient care, with a focus on a safe, secure, and person-centered transition to the next level of care. In your work, you become a key person in the team where commitment, participation, and collaboration are fundamental. Together with doctors, nurses, and other professionals, you contribute with a holistic perspective on the patient's needs and ensure that each individual is seen and heard. You work alongside an experienced colleague, which creates good conditions for a safe introduction and close collegial support in the role. You collaborate closely with municipalities, primary care, home care services, and other external partners. You initiate and participate as needed in coordinated individual planning (SIP). With a strong focus on person-centered care, you ensure that the right services are in place before discharge, and that both patients and families feel secure, well-informed, and involved in the planning. The role also includes documentation in medical records systems, and you serve as important support for the team regarding discharge processes, routines, and collaboration. About the Workplace At Geriatrics, we offer a dynamic and developmental workplace where the care of our frail elderly is at the center. The department consists of 24 care beds, of which 16 are acute care beds and 8 are rehabilitation beds, organized into three teams. Within our acute care beds, we receive, among others, orthopedic patients who, after hip surgery, are transferred to geriatrics for continued recovery and rehabilitation. As a Discharge Coordinator at the Geriatric Department, you may also work at the Rehabilitation Medicine Department to varying degrees. The Rehabilitation Medicine Department has 7 care beds. The Rehabilitation Medicine Department provides care for people of working age with rehabilitation needs following, for example, stroke, traumatic brain injury, or spinal cord injury. You work in a multidisciplinary team with nurses, doctors, nursing assistants, physiotherapists, occupational therapists, speech-language pathologists, and social workers.
