Registered Nurse for Close Care Team Västerås

In collaboration between primary care, specialist care, and municipal health and medical services, we have started a Close Care Team. Our focus is on ensuring that patients in Västerås and Surahammar receive safe and professional care in their homes. We welcome you to a small and pleasant team that appreciates a good laugh. It is important to us that you have adequate time for your assignment so that person-centered care can be created. One of our nurses will be on parental leave next year, and we therefore need to strengthen our team. Does this sound interesting? Please continue reading to get to know us better! Work Tasks Working as a nurse with us is varied work where no two days are the same. You conduct assessments and visits to patients' homes, often in a team together with a doctor or another nurse, but independent visits also occur. Collaboration with home care services and other care providers is central to the work. The nurse in the close care team often functions as a coordinator and connecting link to ensure optimal care for the patient. SIP (coordinated individual plan) is created for most patients. Standard nursing tasks occur. If you have experience and feel confident handling medical-technical tasks such as central lines, that is an asset. Tasks such as intravenous infusions and blood transfusions occur in the role. About the Workplace Close Care Team Västerås was started in the summer of 2022 with the purpose of complementing the care structures that previously existed. Close care means that we in the care structure are positioned between primary care/health centers and specialist care at the hospital. Our goal is to serve as the necessary link so that complex and seriously ill patients in Västerås and Surahammar municipalities can have their care needs optimally met at home. We exist for those who have difficulty accessing care, and our mission is to create security at home to prevent patients from being unnecessarily admitted to the hospital. The team operates from facilities at Ängsgärdet and consists of doctors, nurses, assistant nurses, and a social worker. Interested? Please feel free to contact us if you would like to schedule a no-obligation conversation so we can tell you more. Or would you like to visit us to get an insight into our work and see our facilities? Contact us and we'll arrange it! A driving license is a requirement for the position. As a person, you are confident and stable in your profession, empathetic, and have a natural ability to cooperate with others.
