Duties Duties include home visits, telephone contacts, medication management in the home, simplified medication reviews, suicide risk assessment, ward rounds, development of care plans and safety plans, as well as planning and collaboration with other stakeholders and family members. As a nurse, you are also expected to initiate and follow up on care interventions to promote mental health, prevent inpatient admission, or provide follow-up after discharge.
