Specialist Psychiatric Care Team, SPOT, works directly with patients at a care level between outpatient and inpatient care. The care approach is flexible and person-centered, where patients in a more acute phase can receive daily home visits, assessment, medication treatment, qualified support conversations, and structured suicide prevention. We offer more intensive interventions as a form of intensive outpatient care to prevent inpatient admission and promote earlier discharge. The goal is to work closely with the patient and meet their individual needs. SPOT Norr consists of ten staff members plus a specialist physician. Work is conducted in a team with a coordinator, physicians, nurses, and care assistants, based at the general psychiatry department at Lindesberg Hospital. You will experience collegial community and extensive patient contact. All home visits are conducted together with a colleague. The team currently has two nurses, and we would like to become three! Duties Duties include home visits, telephone contacts, medication management in the home, simplified medication reviews, suicide risk assessment, ward rounds, development of care plans and safety plans, as well as planning and collaboration with other stakeholders and family members. As a nurse, you are also expected to initiate and follow up on care interventions to promote mental health, prevent inpatient admission, or provide follow-up after discharge. When needed, a specialist physician also accompanies home visits. When the patient's condition stabilizes and the team's intervention ends, the patient returns to their regular care contact. Work is conducted during day, evening, and weekend shifts with the possibility of flexible hours. There is an opportunity to combine work in SPOT with work at the outpatient clinic. Qualifications Licensed registered nurse. A specialist nursing degree in psychiatry is desirable. A valid driver's license is required. You have a genuine interest in individuals with mental illness and how teamwork, both internally and externally, along with family members, can support the individual in coping better with their life. You have good self-awareness and are confident as a person and in your role as a nurse with strong ability to make independent assessments.
