Registered Nurse in Advanced Home Care in Malmö

Make a difference. Every day. Do you have a naturally empathetic approach and genuine commitment to patient wellbeing? Do you thrive in a work environment where collaboration, participation, and a positive climate are highly valued? Then you might be our new colleague! Here you'll have the opportunity to work with the patient at the center in their own home environment and be part of a competent, warm, and cohesive team. Together we create security, relief, and quality of life during one of life's most vulnerable phases. The operation is organizationally part of Close Care and Health, Mobile Teams and Specialized Palliative Care, Southwestern Scania. ASIH offers specialized palliative care and other advanced home healthcare to patients of all ages living in Malmö's service area. Care is provided in patients' own homes or in special residential facilities. Patient needs guide the visits; we perform medical and nursing assessments and interventions. We work independently but are part of multiprofessional teams consisting of physicians, nurses, social workers, physiotherapists, occupational therapists, nursing assistants, psychologists, dietitians, and medical secretaries. We have our own on-call line and the operation's physicians are available 24/7. ASIH has full medical responsibility for enrolled patients and works with holistic, person-centered care, which is essential for providing dignified end-of-life care. ASIH provides specialized care but collaborates with home healthcare services when needed and home care services for basic nursing care. We opened three teams in March and have a total of 60 care beds. Additionally, we have an outpatient team consisting of a palliative clinic and a point-of-care team that provides targeted interventions for patients not enrolled in ASIH. 1 position(s). JOB DUTIES Enrolled patients require specialized palliative care, meaning they have a complex symptom picture or their life situation requires special needs. This can range from injections, infusions, medication pumps, catheter placement, ascites drainage, oxygen, and dressings to supportive conversations, family support, or assistance with government contacts and more. All professionals meet in the teams in the morning and plan the day. We primarily conduct scheduled home visits during daytime. Afternoons are used for documentation, reflection rounds, team supervision, individual reflection, and development work. Nurses provide on-call coverage over the teams in evenings and nights. The ASIH phone line is open 24/7 and we continuously assess and make visits as needed for patients requiring it. In addition to daily work, as a nurse you conduct admission meetings with physicians, participate in assessments for potential admissions, and work continuously with family and bereavement support. We work day, evening, and night shifts. As a new nurse with us, you'll receive at least four weeks of on-the-job training. We use an introductory program developed at the unit, and after completing the training period, you'll receive mentorship from a specialist nurse in palliative care tailored to your needs. We value development work and continuing education for both new and experienced staff. ASIH closely collaborates with the palliative care ward, which has 12 inpatient beds. Additionally, we work with the hospital and various clinics. Does this sound exciting? Come visit us and see how we put theory into practice and deliver high-quality, person-centered care daily. QUALIFICATIONS We are looking for a licensed nurse with authorization to work in Swedish healthcare, preferably with several years of professional experience. Swedish language proficiency at least at level C1 according to the Council of Europe's scale is required. Since the operation consists of care beds in patients' own homes or special residential facilities, a valid B driving license for manual transmission cars is required for the position. We are pleased if you have specialist training in palliative and/or oncology care. You are confident in your professional role and keep the patient in focus, which means you can see the patient as an individual with their own needs. Part of the work involves support and conversation, so it's important that you are communicative and able to meet people on different levels. Furthermore, you have a flexible approach and can prioritize different tasks when the pace of work is high. To thrive with us, you need to be able to work independently and make your own decisions. We appreciate if you are willing to share your knowledge and good humor to create a positive work environment. We value a positive attitude and a willingness to participate in our development work. As you will become an important colleague in our operation, we appreciate good teamwork ability and personal suitability. In this recruitment, we use continuous selection. Approximately 3,000 employees work with us every day to provide good and safe specialist healthcare of the highest quality. Do you want to develop in an operation with broad medical expertise? Do you want to work close to the acute patient flow, make qualified clinical assessments, and at the same time develop in an operation with broad medical expertise? On Medical Unit 4, you will meet patients with everything from acute medical conditions and infections to cardiac, pulmonary, and surgical issues. The breadth means that no two days are alike, and you quickly develop strong clinical competence. We are a relatively young operation that continues to develop. This means you have the opportunity to influence working methods, routines, and future development, while also receiving support from experienced colleagues and a clear commitment to creating a safe and learning work environment. With us, there is a senior nurse function with clear clinical presence in daily work. The function is part of our systematic work with patient safety, learning, and competency development, and provides security for new colleagues as well as support and deepening for more experienced nurses. About Medical Unit 4 Medical Unit 4 is a broad internal medicine ward with 28 care beds where acute care and inpatient care work together. The unit encompasses both the emergency care unit (AVA) and traditional medical inpatient care, which creates a unique breadth and variation in daily work. We care for patients with a wide spectrum of medical diagnoses and, in the AVA, also surgical diagnoses. Many patients are early in their care journey and require careful clinical assessments, observations, and quick decisions, while others need continued medical treatment and care. This means that as a nurse, you develop both your acute competence and your ability to plan and coordinate care over time. Work is characterized by variation, flexibility, and the opportunity to gradually broaden or deepen your competence based on interest and experience. An operation in constant development Development is a natural part of our daily work. We continuously work to improve patient flows, working methods, and competency development to meet future care needs. We also have close collaboration with the hospital's other medical operations, including HIA and IMA, which creates good opportunities to broaden and deepen your competence within several different care levels. With us, you should be able to grow in your profession without having to leave the operation. Your role as a nurse With us, you work in a team together with doctors, nursing assistants, and senior nurses, in person-centered care with a focus on quality, patient safety, and good collaboration. Work is characterized by: - Acute and subacute care flows - Medical assessments and observations - Fast care processes where the right decision at the right time is crucial - Care of patients with complex care needs - Teamwork with shared responsibility for planning, implementing, and following up on care With us, you get the best of both worlds – the pace and clinical challenges of acute care, combined with the opportunity to follow the patient's continued care journey. This creates a unique work environment where you develop both broad medical competence and strong clinical judgment skills. Who are we looking for? We are looking for you who are a registered nurse and who: - Thrive in an environment with pace and variation - Want to work close to acute care but in an organized and safe context - Are curious, responsible, and like to learn new things - Appreciate collaboration and teamwork - Want to contribute to the development of the operation You can be newly graduated or have several years of experience. With us, it is just as important how you want to work as how long you have been doing it. Great emphasis is placed on personal suitability. Development and education We want you to feel safe in your role and continue to develop over time. Therefore, we offer: - Individually customized orientation (4-5 weeks) - Frequent follow-ups with the unit manager - Support from senior nurse in daily work - Regular internal training in medicine and care - Clinical foundation year for newly graduated nurses - Simulator training and cross-organizational learning Development with us is not a project on the side – it is part of everyday work.
