High Availability Aftercare for Youth at Risk of Criminal Involvement means that a special team consisting of family treatment specialists has been established in the relevant district administrations. The teams' main mission is to create conditions for a socially functioning life for youth leaving state care by offering coordinated and qualified support. The support is designed based on the youth's and parents' needs and is characterized by a high degree of flexibility regarding choice of method, intensity, and meetings with consideration to time, place, and form. The intervention is distinguished by high availability. To ensure flexibility and rapid interventions linked to needs, family treatment specialists and social workers exercising authority work closely together, among other things through frequent and regular check-ins. Your Role You work as a family treatment specialist and provide a structured aftercare intervention to youth leaving state care. The intervention includes different phases for planning, implementation, and maintenance and constitutes a comprehensive solution where social interventions are implemented at different levels in parallel. You work with the youth individually through behavior and skills training, with communication and interaction in the family from a systemic perspective, and with mobilizing and coordinating a prosocial network with both private contacts and professional actors.
