Your Workplace Department 9 is a geriatric internal medicine ward specializing in elderly patients. Diseases treated here include COPD, heart failure, diabetes, and infections. Many patients have a combination of multiple diseases, which we are particularly skilled at managing. We work person-centered with care that takes into account the complex care needs that arise from multimorbidity and natural aging. Patients come to us through the emergency department, other care units, or as direct admissions via mobile teams. The department works interprofessionally and team-based with and around the patient according to CGA (comprehensive geriatric assessment), which means structured care of the elderly patient. Twice a week, team-based rounds take place to establish a frailty diagnosis, which then forms the basis for treatment level, testing, and care. We have a receptionist and a discharge coordinator nurse who provide administrative support. This relieves the nurse in their work and ensures a safe and secure discharge for the patient.
