Diabetes Nurse / Nurse at Borås Primary Care Center

Borås Primary Care Center is seeking a licensed nurse with advanced training in diabetes care. The position is flexible, ranging from one day per week to full-time. We are now seeking a diabetes nurse who wants to lead clinic operations and, together with the rest of the team, continue to develop diabetes care at the primary care center. Your Assignment As a diabetes nurse, you will work independently with your own clinic, primarily serving patients with type 2 diabetes. Your work includes scheduled diabetes visits and annual check-ups, counseling, treatment and follow-up, patient education and support for self-care and lifestyle changes, monitoring of medication treatment and dose adjustments according to prescription, treatment plans, and facility procedures, registration and quality monitoring in the National Diabetes Registry (NDR), and collaboration with physicians and other professionals regarding patient treatment. You will also coordinate with specialized care when the patient's diabetes treatment requires it. You work independently in your clinic but have access to physicians at the primary care center for medical assessment and joint handling when needed. Work is conducted according to current national and regional knowledge support and the requirements applicable to primary care centers within VGR Vårdval. If You Want to Work More If you want to work more than the time needed for the diabetes clinic, the position can be combined with other nursing work at the primary care center.
