Your work tasks include: • Receive applications under SoL and investigate the individual's need for support. • Make decisions on services or denials based on the results of the investigation. • Collaborate with customers, relatives, and other professional actors. • Follow up and evaluate services to ensure high quality. • Work independently with your own cases, but also closely with colleagues in the team. As a case manager, work may be assigned within the team where there are needs in the operations. QUALIFICATIONS Who are you? REQUIREMENTS We are looking for someone who has: • A degree in social work, behavioral science, or other education that the employer deems equivalent. • A B-class driver's license.
