Together, we create safe and high-quality care and social services for our residents. We are now seeking a substitute for a one-year role as a case manager at the Social Services Unit. 1 position(s). WORK TASKS As a case manager, you work close to people in different life situations and help design interventions that make a real difference for our residents. The work is both independent and team-based, and you will use your expertise to find solutions that are both sustainable and legally sound. In the Social Services team, we are divided into different geographical areas, where you are responsible for receiving, investigating, and assessing incoming applications for support and services under the Social Services Act (SoL). Your work tasks include: • Receive applications under SoL and investigate the individual's need for support. • Make decisions on services or denials based on the results of the investigation. • Collaborate with customers, relatives, and other professional actors. • Follow up and evaluate services to ensure high quality. • Work independently with your own cases, but also closely with colleagues in the team. As a case manager, work may be assigned within the team where there are needs in the operations. QUALIFICATIONS Who are you? REQUIREMENTS We are looking for someone who has: • A degree in social work, behavioral science, or other education that the employer deems equivalent. • A B-class driver's license. QUALIFICATIONS • Previous experience as a case manager.
