The team can assess, plan, prescribe, and deliver interventions. Care is specialized based on each patient's individual needs, and the goal is to provide patients with qualified care both medically and psychosocially. RESPONSIBILITIES Your role involves working in a team where you make home visits to patients in the various municipalities.
