You will be an important team player working closely with other professionals. You will need to be available by phone to support nurses in HSV and SÄBO. You will also participate in the health center's dementia team in connection with dementia assessments and collaborate with our heart failure nurses. The average length of stay is approximately one week. The Primary Care Departments work closely together with a primary care perspective. Ensuring that our patients' care needs are met even after they leave the department is a very important part of our work. Therefore, we collaborate with, for example, Mobile Primary Care Teams, regular primary care, home health care, and others in the network that create value for the patient. Here we have more time for each patient and work according to a patient-centered care model.
