Patients requiring advanced interventions receive care in their homes. The work is based on a multidisciplinary team consisting of Doctors, nurses, nursing assistants, physiotherapist, occupational therapist, dietitian, and social worker. We work person-centered and play a clear role in the care pathway in relation to other healthcare providers. As a nurse, you perform medical interventions in the patient's home such as nutritional treatment, hydration, infection treatment, pain relief, end-of-life care. Several patients spend their final days at home. The work also involves coordinating care with specialist clinics throughout Stockholm, such as our oncology clinics, by preparing patients for various treatments including blood tests and transfusions. You will plan, perform, and evaluate patient care. You will work independently by making your own assessments in the home and performing interventions according to guidelines and in consultation with the Doctor. The Workplace ASIH Tyresö/Söderort is a small healthcare provider working on behalf of Region Stockholm since October 2015. We are an organization that works close to patients with short decision-making paths. We offer individually tailored onboarding with a mentor during the first 6 months. Training in palliative care at PKC during work time. Palliative training 7.5 credits paid after 1 year of employment. We have a mobile PICC line team at the unit. They place PICC lines in homes and hospitals, we also have a PICC line clinic in Solna and Tyresö. We have a couple of innovation projects in the operations.
