As a rehabilitation coordinator you will: * Support patients on sick leave in the rehabilitation process and facilitate their return to work. * Conduct mapping conversations and establish rehabilitation plans in dialogue with the patient, treating physician, and other stakeholders. * Participate in collaboration with the health center's doctors, psychosocial team, and other staff regarding complex cases. * Coordinate contacts with external actors such as the Swedish Social Insurance Agency, the Swedish Employment Agency, and employers. * Conduct multi-party meetings and follow up on the patient's path back to work or other activities.
