Fokus Liberec, an organization operating in the Liberec region for 30 years, is seeking a colleague for the position of social worker in a multidisciplinary Mental Health Center team for people with mental illness. This work is suitable for anyone interested in mental health, with the ability to make independent decisions and a willingness to expand their knowledge and competencies. Job Description: The worker provides social work in accordance with Act 108/2006 Sb in the Mental Health Center service. This is dynamic, responsible, and important work with people with mental illness and their surroundings, based on multidisciplinary collaboration. The worker serves in the role of a key worker, responsible for coordinating and providing services to specific clients. In collaboration with the multidisciplinary team, the worker supports clients in addressing and managing difficult life situations, establishes rehabilitation procedures with consideration for the client's individual capabilities, communicates with cooperating entities, provides basic social and legal counseling, and maintains client documentation. We expect the worker to proactively collaborate in developing procedures, evaluating service quality and delivery methods, planning service goals, and assisting in service presentation.
