Job Description Your task is to work in a coordinated manner for patients on sick leave and where care needs to be coordinated with other parties such as employers, the Social Insurance Agency, the Employment Service, or municipal social services. In your role as rehabilitation coordinator, your tasks are to provide support and advice to individual patients and conduct assessments of individuals' rehabilitation needs. You will be responsible for and participate in SIP/care plans and meetings with employers. Your duties include supporting doctors and working together to create clear rehabilitation plans. An important part of the assignment is to coordinate and cooperate both internally and externally, all to support the patient and help prevent and shorten sick leave periods. Work Team We are approximately 35 employees at the health center who enjoy working together in an open work environment. As a rehabilitation coordinator with us, you work in close collaboration with the other staff members of the health center. The health center has a doctor's office, specialist nurse clinics, nurse clinic, child health care, psychosocial expertise, foot care, and laboratory. We also manage a number of nursing homes. About You You are trained as a physiotherapist, occupational therapist, nurse, or have other education and experience that we consider equivalent. Experience in primary care and previous work with rehabilitation issues and care coordination is a merit. We would like to see that you have experience working in a team and that you are familiar with our medical records system Cosmic. Furthermore, you must have fluent Swedish in speech and writing, corresponding to C1 level. For you to thrive and fit in with us, your personal qualities are important. We are looking for someone with a high degree of personal maturity. You have good collaboration skills but are also independent in your way of working. You can make professional assessments, initiate and conclude treatments, and have a good ability to structure your work. The workload is normally around 10-30 hours per month but can vary over time.
