Job Description Your task is to work in a coordinated manner for patients on sick leave and where care needs to be coordinated with other parties such as employers, the Social Insurance Agency, the Employment Service, or municipal social services. In your role as rehabilitation coordinator, your tasks are to provide support and advice to individual patients and conduct assessments of individuals' rehabilitation needs. You will be responsible for and participate in SIP/care plans and meetings with employers. Your duties include supporting doctors and working together to create clear rehabilitation plans. An important part of the assignment is to coordinate and cooperate both internally and externally, all to support the patient and help prevent and shorten sick leave periods. Work Team We are approximately 35 employees at the health center who enjoy working together in an open work environment. The company was founded in France in 2013 and relocated to Linköping in 2020.
