The Assignment The position is based during daytime hours, Monday-Friday. The role includes planning, coordinating and implementing care services, communication with other healthcare providers and home services, as well as direct and telephone contact with patients and their families. Background We are looking for a licensed nurse. You need to have at least two years of experience as a nurse, preferably from a health center or home care, or have worked as a nursing assistant in home care before becoming a licensed nurse. It is important that you are someone who feels committed to your profession. It is important that you have good computer skills, preferably with experience using the Take Care and Lifecare systems. You are required to have a strong command of Swedish, both spoken and written. You work independently and person-centered while being an important part of a multidisciplinary team together with nursing assistants and rehabilitation staff. Your duties include, among other things: Health and nursing assessments and follow-ups Medication management and delegation Planning, implementation, and evaluation of care interventions Cooperation with doctors, rehabilitation teams, and care personnel Documentation according to applicable guidelines Support and guidance to nursing staff Contact with relatives and other care providers The position is hourly employment and working hours are Monday through Sunday during daytime.
