The Role As a nurse in Pediatrics and Adolescent Medicine at Aleris Specialist Clinic Bäckefors, you will meet patients with many different diagnoses. The clinic receives children and adolescents ages 0-18 and the assignment encompasses medical assessment, treatment, and patient care. Patients come to us through referral from other healthcare providers or through self-referral. Here we work closely with individuals and put their needs at the center – always with a focus on security and quality. About Us The case management unit is part of individual and family services and is responsible for government administration according to the Social Services Act within care and support for residents in Bengtsfors and Mellerud Municipality. Since 2023, we have been based at our office at Dalslands Hospital in Bäckefors. We are in an exciting development phase with new working methods and team structure, as well as a senior reception. With us, you will receive collegial support, reflective team sessions, and a work environment where we help each other. We have a senior case officer who leads and distributes work through individual check-ins, joint case supervision, and methodological support. Your Role As a case officer, you investigate and make decisions about the right to services according to the Social Services Act, such as home care, special housing (care homes), or short-term stays. You are also responsible for following up and evaluating whether the help is working as intended. We conduct investigations using the IBIC methodology. You will meet people in different life situations, collaborate with relatives and other professionals, and contribute to a safe care transition. The work requires structure, legal certainty, and good communication – both verbally and in writing. Your duties as a case officer include, among others: Receiving applications according to the Social Services Act, investigating and based on the results of your investigation, making a decision about appropriate services or a decision of rejection.
