Inform and educate clients on issues related to their obligations and rights, as well as the procedure for receiving social services. Ensure, at least once a year, assessment of client functional status and develop an individual social care or social rehabilitation plan in cooperation with social work specialists, according to client social care or social rehabilitation needs. Ensure transparent recording of information in client files about the progress of the social care process and make entries at least 1x per week in client files about the progress of the social rehabilitation plan. Implement client individual social care or social rehabilitation plans, modify and supplement them according to changes in client life situations. Assist clients in adapting to social and physical environment, promote integration into society. Assess client personality, self-care abilities, physical and mental health status. Monitor changes in mental state, behavior and health status of clients in care. Inform on-duty nurses, psychiatrists of changes and make entries in client file. Prepare and complete work documentation according to regulatory requirements and procedures established in the institution.
