Psychiatric Hospital in Opava is seeking a social worker for a newly established service - Mental Health Center who wants to be part of a multidisciplinary team providing support to people with mental illness. Expected job responsibilities: - providing social work and professional social counseling to clients, - social assessment and development of individual support plans, - supporting clients in addressing their social situation, - collaboration with family members and institutions, - maintaining documentation in accordance with legal regulations and quality standards. Requirements: - professional qualification to practice as a social worker pursuant to § 110 of Act No. 108/2006 Coll., on social services, as amended, - clean criminal record, - health fitness, - responsibility, independence, empathy, communication skills, - ability to work independently and as part of a team, - respectful and individualized approach, - ability to maintain electronic documentation related to service provision, - Group B driving license - active driver (advantageous), - experience in caring for people with mental illness or in community services (advantageous). Job position characteristics: - full-time employment, - salary classification in the 11th pay grade according to Act No. 262/2006 Coll., Labor Code, Government Regulation No. 341/2017 Coll., and length of creditable experience, - 5 weeks of vacation, - additional employee benefits provided by Psychiatric Hospital in Opava. Please send job applications to: [email protected] JOB RESPONSIBILITIES: - The social worker performs social assessments according to §108 of the Social Services Act, manages social affairs including resolution of socio-legal problems in facilities providing social care services, provides socio-legal counseling, performs analytical, methodological and conceptual activities in the social field, and performs professional activities in facilities providing social services. - Provides basic social counseling and handles applications for care allowances. - Communicates with service applicants and conducts social assessments. - Maintains required client documentation within their competence, drafts contracts and amendments for the provision of social services with clients. - Processes payments for accommodation, meals, care, health insurance premiums (SIPO), medications, rehabilitation and other payments from clients. - Creates and collaborates on the development and implementation of quality standards, provides suggestions for improvements and recommendations to enhance service quality.
