Description: We are looking for a district nurse who wants to join our home health care team. Our team consists of 14 colleagues distributed across 2 district nurses, 6 registered nurses, 6 nursing assistants, with two RNs serving as coordinators and team leaders. The team also includes a physician who provides full-time home health care. Together we care for 240 home health care patients. Together with nursing assistants, you perform planned or acute health care interventions in patients' homes. As a district nurse, you have nursing responsibility for patients in your area. This includes medical assessments, wound care, medication management, laboratory sampling, catheter care, nursing planning, care planning via SIP (Coordinated Individual Plan), contact with family members, benefits assessors, and prescriptions.
