Description:
We are looking for a district nurse who wants to join our home health care team. Our team consists of 14 colleagues distributed across 2 district nurses, 6 registered nurses, 6 nursing assistants, with two RNs serving as coordinators and team leaders. The team also includes a physician who provides full-time home health care.
Together we care for 240 home health care patients. We value flexible and close collaboration between our professional groups to best serve our patients. You work in a team, but in the field you typically work independently.
Ekerö Health Center is one of Stockholm's largest health centers with 20,000 registered patients. We are a large fully staffed workplace with nearly 60 employees.
In addition to home health care, we have various clinics including diabetes, asthma/COPD, hypertension, travel vaccinations, and a psychosocial team with psychologists and a rehabilitation coordinator.
We are located in Ekerö Center, easily accessible by bus from Brommaplan, ferry or bus from Slagsta, or Boat 89 from downtown.
Duties
The home health care team is divided into 3 areas, with each area consisting of 2 nursing assistants and 2-3 RNs/district nurses. Our patients live on Lovö-Drottningholm, Ekerö, Munsö, and Adelsö. We visit our patients on foot, by bicycle, and by car. Together with nursing assistants, you perform planned or acute health care interventions in patients' homes. As a district nurse, you have nursing responsibility for patients in your area. This includes medical assessments, wound care, medication management, laboratory sampling, catheter care, nursing planning, care planning via SIP (Coordinated Individual Plan), contact with family members, benefits assessors, and prescriptions.
You play a central role in patient coordination, ensuring high quality nursing care and patient follow-up. You lead, prioritize, and coordinate work around your patients.
Qualifications
District nurse with several years of work experience, preferably from home health care and/or geriatrics. A valid driver's license (category B) is required. We use Take Care for documentation, and experience with this system is advantageous.

We are looking for someone who is confident and stable, with a professional approach and the ability to adapt to different situations. You are skilled at creating structure and have a natural talent for planning, organizing, and prioritizing your work efficiently. You are positive, helpful, and can work independently while also collaborating well with others.
Additional Information:
We are primarily seeking a full-time position, but part-time can be discussed. Wellness allowance 5,000 kr per year. Collective agreement, vacation accommodations.
