Your job responsibilities include: • conducting medical assessments and monitoring the patient's health status • planning, implementing, and following up care interventions • working preventatively to promote health, well-being, and quality of life • performing, for example, blood draws, infusions, and dressing changes, as well as providing end-of-life care • documenting and registering in quality registers and contributing to developing and ensuring care quality • guiding and delegating care personnel and students • collaborating with internal and external care providers and other partners. Patient rounds take place together with doctors from the registered health center. HSL documentation is done in Treserva and planning is done in TES. Lifecare is used for communication between care providers. The position is mainly based during daytime, but evening and weekend shifts are included. During these shifts, you work in the municipality's consultant operations, which means you may work in the municipality's care and housing facilities, home care services, as well as service and group homes within LSS and social psychiatry. QUALIFICATIONS We are looking for someone who is confident in their profession and who wants to contribute with both your nursing expertise and your commitment. For the position, we require: • nursing license • at least two years of experience working as a nurse • a B driver's license and good driving skills • good knowledge of Swedish in speaking, writing, and reading comprehension • good documentation habits. It is advantageous if you have experience in municipal health and medical care and with the systems Treserva, TES, Lifecare, NPÖ, and Pascal. Background checks will also be conducted via the National Board of Health and Welfare's HOSP register and IVO, the Inspectorate for Care Services, prior to employment.
