The Unit for Harmful Substance Use, Addiction, and Social Psychiatry is part of the Social Services Department and provides services for adults with harmful substance use, addiction, and/or psychosocial problems. The unit encompasses activities such as supported housing for adults, co-morbidity housing, accommodations with special services, addiction units, and residential support. The Assignment Many clients have complex problems with various diagnoses, mental health issues, and active harmful substance use and addiction, and are unable to meet the requirements needed to move forward in the process. This could involve obtaining mobile banking ID, maintaining healthcare contacts, attending scheduled appointments, etc. These individuals need support and assistance in removing obstacles, navigating counterforces/risks such as relapse, interrupted medication, and similar challenges. The Care and Support Coordinator has a special assignment to guide and support the client practically. The overarching goal is to shorten the length of stay in supported housing, short-term placements, or to reduce the number of recurring, short placements and treatments, and to achieve a more sustainable solution in the long term. Within the framework of assistance decisions, you work in close collaboration with colleagues and placing social workers. 1 position(s). WORK TASKS As a Care and Support Coordinator, your task is to map and make accessible the support the client needs, while also providing practical support yourself. To succeed, you need good perseverance and very good collaboration and initiative skills, and you must be able to independently plan, implement, and follow up on interventions. Practical support may include, for example, making phone contacts and accompanying the client to physical meetings. You support the client in contact with various authorities, healthcare, and other necessary contacts. You assist in obtaining documents and records. You need to support/help the client gain an understanding of society. In supporting the client to find strategies and opportunities, Motivational Interviewing is the guiding approach. To succeed in your work, you need to establish good collaboration and cooperation internally and externally, and be flexible in your approach. As a Care and Support Coordinator, you are expected to participate in development work and have an understanding of what it means to work in an organization undergoing development. You believe that change is possible and can convey calm and a positive feeling to clients even during difficult circumstances.
