You will be based at our office in Jönköping and be part of our well-established team where we together conduct inspections across Småland, Öland, and parts of Blekinge. By conducting inspections of lifting equipment such as elevators, mobile work platforms, vehicle lifts, and more, you will help our customers maintain a high level of safety. What many often find best about working with lift equipment inspections is the variety, the challenges, and the many customer interactions! Job Description Your assignment is to work in coordination for patients who are on sick leave and where care needs to be coordinated with other parties such as employers, the Social Insurance Agency, the Employment Service, or municipal social services. In your role as Rehabilitation Coordinator, your tasks include providing support and advice to individual patients and conducting assessments of individuals' rehabilitation needs. You will be responsible for and participate in rehabilitation plans and meetings with employers. Your work tasks include supporting doctors and together creating clear rehabilitation plans. An important part of the assignment is to coordinate and collaborate both internally and externally, all to support the patient and help prevent and shorten periods of sick leave. Work Team We are approximately 35 employees at the health center who enjoy working together in an open work environment. As a rehabilitation coordinator with us, you work in close collaboration with the health center's other staff. At the health center there is a doctor's office, specialist nurse clinics, nurse clinic, child health care, psychosocial expertise, foot care, and laboratory. We are also responsible for a number of care facilities. About You You are educated as a physiotherapist, occupational therapist, nurse, or have other education and experience that we deem equivalent. It is advantageous to have experience in primary care and to have previously worked with rehabilitation issues and care coordination. We would like to see that you have experience working in teams and that you are familiar with our journal system Cosmic. Furthermore, you must master Swedish language in speech and writing, corresponding to C1 level. For you to thrive and fit in with us, your personal qualities are important. We are looking for someone with a high degree of personal maturity. You have good collaboration skills but are also independent in your way of working. You can make professional assessments, initiate and conclude treatments, and have a good ability to structure your work. Psychologist at Habilitation Clinic, Region Västernorrland

About the Operations Regional Habilitation Services in Region Västernorrland consists of approximately 100 employees located in Sollefteå, Sundsvall, and Örnsköldsvik. We work in multidisciplinary teams with medical, neuropsychological, social, and educational expertise to meet children, adolescents, and adults with lasting functional disabilities. Our largest target groups are individuals with intellectual disabilities, autism, and cerebral palsy. At the habilitation clinic in Sollefteå, there are two psychologist positions—one in the children's team and one in a combined adolescent/adult team. We are now seeking a new colleague for the adolescent/adult team. With us, you get a workplace with close collaboration between professions. The team includes occupational therapists, physiotherapists, social workers, speech-language pathologists, and psychologists, and we also collaborate with habilitation physicians and nurses. You will have access to both internal and external supervision as well as opportunities for further education. Choice of work location (Sollefteå, Sundsvall, or Örnsköldsvik), but the work will be conducted at the Habilitation Clinic in Sollefteå.
