At the Rehabilitation Unit, we work in close collaboration with other staff in care and support services within the municipality's special housing facilities, short-term housing, home health care, and home services, as well as for individuals who receive services under LSS. The Rehabilitation Unit serves the entire Härjedalen region and operates from locations in Funäsdalen, Hede, and Sveg. Work Location: Sveg Responsibilities - Provide good and safe care throughout all stages of life and identify and actively prevent health risks. - Conduct physical therapy assessments and follow-ups based on the individual's needs and circumstances. - Maintain close, team-based collaboration with occupational therapists, nurses, benefits coordinators, care staff, and family members. - Documentation in the journal systems Viva and Cosmic. The work requires independence as you will structure and plan your working days on your own. Working hours are daytime, weekdays.
