Our day begins at 07:00 in the morning and ends at 21:00 every day of the week, during which time our night staff is on duty. All care staff work both days and evenings as well as weekdays and weekends. We have no full-day shifts or split shifts. Sjöberga Gård is a specialized residential facility in Hässelby with a focus primarily on dementia diagnoses including frontotemporal dementia, Korsakoff's syndrome, and Lewy Body dementia. At Sjöberga Gård, we have twelve apartments on two floors. On each floor, we have a common dining room and a day room as well as a kitchen. All meals are prepared in-house. We have a patio at ground level and a balcony on the second floor. We also have dogs that are on duty sometimes when their owner/handler is working. We are a smoke-free workplace. As a care staff member, your most important task is to ensure that each person receives the care and support they need. As an occupational therapist in Primary Care Rehabilitation, you meet patients of all ages and with different types of diagnoses. You meet patients in their homes or at our clinic, individually or as part of a team. With us, you as an occupational therapist are an important part in the rehabilitation of our patients. Team Aktiv Primary Care Rehabilitation has since 1995 provided rehabilitation services to the population in Hässelby-Vällingby. With us work 3 occupational therapists, 2 dietitians and 8 physiotherapists who together develop and are responsible for the operations. Job duties include: Activity and function assessment/investigation/training, provide advice and support on self-care. Perform dementia assessment as part of basic dementia investigation in collaboration with the general practitioner. Prescribe assistive devices and provide information about products and technology. Assessment of housing and surrounding environment and environmentally-focused interventions such as home modifications. Hand rehabilitation individually or in group treatment such as Osteoarthritis School. Collaborate with other healthcare providers, mentor and support family members and care staff, and guide students. Documentation is done in the Take Care journal system. Qualifications You must be a licensed occupational therapist with Swedish credentials. It is advantageous if you have worked in Primary Care, have experience documenting in the Take Care journal system, and have good knowledge of Swedish and English. You must have a Class B driver's license (manual transmission). Since we work in patients' homes, you must be comfortable in environments with pets/animals and not be sensitive to smoke odors.
