WORK TASKS Since one of our nurses will be enjoying the arrival of a baby this winter, we are now looking for a responsible nurse for our HSL team! The maternity leave coverage extends until the end of August 2027. Together, we form a good team of permanent staff consisting of evening/night, day, and resource nurses, cognitive support team, occupational therapists, physiotherapists, rehabilitation assistants, activity developer/system responsible, managers, and operations manager. Team collaboration with nursing care staff, unit manager, and rehabilitation. Close collaboration with the other nursing care coordinator at the residential facility as well as the nurse in home care services. RESPONSIBILITIES Working as a district nurse/nurse at Bräcke Health Center involves traditional health center tasks such as patient consultations, phone advice, and teamwork with physicians and other staff. The work is also very varied and includes acute tasks. As a coordination nurse, you handle our daily communication with the municipality and hospital via LINK, you are responsible for coordinated individual care plans (SIP), and are available on a dedicated phone line for vulnerable patients. You assist the team with physician contacts, conduct home visits for MMSE, SMA, or other interventions. You serve as the hub for expanded primary care at Bräcke Health Center. Initially, the work will consist of 50% as a coordination nurse and 50% at the health center; this may change if the number of patients for the coordination nurse increases. You may base yourself in Bräcke or Kälarne, according to scheduling. Based on your assignment, you have great flexibility in planning your work. QUALIFICATIONS Licensed nurse or further education as a district nurse or in elderly care. You have a genuine interest in people and patient-focused approach.
