Image and Functional Medicine (BFM) Västerbotten operates extensive activities at Norrland University Hospital in Umeå, Skellefteå Hospital, and Lycksele Hospital, as well as at health centers in southern Lapland. Family Caregiver Consultant, Prevention Unit, Elderly Care Services

Many adults in Sweden care for, help, or support a close relative at some point in their lives who is chronically ill, elderly, or has a disability. When this occurs regularly, more than once a month, it is referred to as family caregiving or family care. The Prevention Unit offers and provides support to family caregivers for their own wellbeing. The goal is to improve quality of life, reduce the burden on caregivers, and minimize the risk of caregivers experiencing health problems themselves. Support is available for family caregivers whose relatives are adults living independently. Currently, family caregivers can access support in the form of counseling, health-promoting activities, caregiver training, and flexible relief care. Paid family caregiving is managed by the family caregiver consultant within Prevention. We are now looking for someone who would like to work as a temporary family caregiver consultant with us! Your Responsibilities As a family caregiver consultant, you offer support through individual conversations, information, advice, and guidance based on the caregiver's situation and needs. You plan and conduct training sessions, lectures, and activities, and handle and manage paid family caregiving. An important part of the role is to contribute to the development of the organization based on the national caregiver strategy. An essential part of your role is to collaborate with both internal and external partners, such as social workers, the Senior Square, volunteer coordinators, advocacy organizations, and health and medical services. You help family caregivers find the right support and often serve as a guide to other services when needed. About You You naturally build trusting relationships with family caregivers, their relatives, colleagues, and collaborative partners. You have a genuine commitment to family caregiver support and meet people with attentiveness, respect, and empathy. As a person, you are service-oriented, willing to take initiative, and actively contribute to the organization's development. You have good collaborative skills and thrive working both together with others and independently toward common goals. You are receptive in collaboration with others while being willing to take initiative, express your opinions, and communicate in a clear and constructive manner. Coordinator for the Unit for Digital Care and Health Services

About Us The Unit for Digital Care and Health Services is responsible for installing digital solutions in homes and supporting people in using digital technology. We already have IT technicians who work practically with installation, service, and support. As a coordinator, you will work closely with the technicians and together with other roles in the team drive the development of this new branch forward. The unit is now evolving from a technology focus to leading, coordinating, and delivering digital initiatives and solutions for the entire elderly care service. Our digitalization team, which you will become part of, consists of business developers, project managers, coordinators, and IT technicians. Together, we work both strategically and practically to create solutions that work in everyday life. We believe in technology that creates real value for both citizens and employees, and in collaboration as a key to success. We are now recruiting a substitute coordinator (social worker) to our team! You will have a key role in building an innovative, multiprofessional operation where care, technology, and collaboration meet to create a better everyday life for older people. Here you will be an important link between the individual's needs and the digital solutions that provide security, independence, and quality of life. Working hours may vary based on operational needs and may include work outside office hours. Responsibilities Participate in building our team where you will implement and develop processes within the unit. Assess and match digital solutions to the individual's needs and wishes within care and health services. Collaborate with the unit's IT technicians to create functional comprehensive solutions for the individual. Actively collaborate with other professionals and authorities to develop operations and digital initiatives. Identify when digital solutions are no longer sufficient and escalate the need to benefits officers. Meet people in vulnerable situations and provide professional support to individuals and relatives throughout the process. Guide colleagues who deliver digital care and be a change leader in implementing new working methods. Perform administration and documentation related to the individual. Help shape procedures, working methods, and digital solutions for future care. Work according to procedures based on the Social Services Act, Public Access to Information and Secrecy Act, and administrative law requirements. Coordinate the housing placement process in close collaboration with benefits officers and other stakeholders. Offer available housing placements and inform the individual and relatives before moving in. Follow up and document housing placements according to current procedures and legislation. Collaborate within the digitalization team and with other professionals. Guide colleagues and participate in development and improvement work. Contribute to innovative solutions and the development of future elderly care. Qualifications Degree in social work or equivalent university education. Good knowledge of the Social Services Act, public access to information and secrecy, and administrative law. Experience in case management and professional interaction with people in vulnerable situations. Good digital competence, including the ability to independently and efficiently handle relevant systems, digital tools, and platforms. Ability to lead, collaborate, and drive change initiatives in an organization. Strong interest in digital development and innovation within care and health services. Good ability to express yourself in writing in Swedish as the work includes writing clear, structured, and appropriate reports, documents, and documentation. Good knowledge of Swedish in speech and writing, and good knowledge of English. Class B driver's license as the position includes home visits. We consider it meritorious if you have experience in project management and if you have training in project management, investigation, work supervision, and digitalization.
