We work independently but are part of multiprofessional teams consisting of physicians, nurses, social workers, physiotherapists, occupational therapists, nursing assistants, psychologists, dietitians, and medical secretaries. We have our own on-call line and the operation's physicians are available 24/7. ASIH has full medical responsibility for enrolled patients and works with holistic, person-centered care, which is essential for providing dignified end-of-life care. ASIH provides specialized care but collaborates with home healthcare services when needed and home care services for basic nursing care. We opened three teams in March and have a total of 60 care beds. Additionally, we have an outpatient team consisting of a palliative clinic and a point-of-care team that provides targeted interventions for patients not enrolled in ASIH. 1 position(s). JOB DUTIES Enrolled patients require specialized palliative care, meaning they have a complex symptom picture or their life situation requires special needs. This can range from injections, infusions, medication pumps, catheter placement, ascites drainage, oxygen, and dressings to supportive conversations, family support, or assistance with government contacts and more. All professionals meet in the teams in the morning and plan the day. We primarily conduct scheduled home visits during daytime. Afternoons are used for documentation, reflection rounds, team supervision, individual reflection, and development work. Nurses provide on-call coverage over the teams in evenings and nights. The ASIH phone line is open 24/7 and we continuously assess and make visits as needed for patients requiring it. In addition to daily work, as a nurse you conduct admission meetings with physicians, participate in assessments for potential admissions, and work continuously with family and bereavement support. We work day, evening, and night shifts. As a new nurse with us, you'll receive at least four weeks of on-the-job training. We use an introductory program developed at the unit, and after completing the training period, you'll receive mentorship from a specialist nurse in palliative care tailored to your needs. We value development work and continuing education for both new and experienced staff. ASIH closely collaborates with the palliative care ward, which has 12 inpatient beds. QUALIFICATIONS We are looking for a licensed nurse with authorization to work in Swedish healthcare, preferably with several years of professional experience. Swedish language proficiency at least at level C1 according to the Council of Europe's scale is required.
