Main job duties: - Identify social problems and determine client needs; - Develop intervention plans (in cooperation with clients) according to identified social problems with the aim of improving or developing the client's social skills, motivating change; - Provide clients with psychosocial assistance and individual consultations on social, educational, legal and other matters within the scope of the position; - Organize client access to social services at their place of residence, crisis centers, social rehabilitation institutions, long-term social care and social rehabilitation institutions, including out-of-home care facilities and institutions; - Prepare response documents within the scope of competence for natural and legal persons and issue, including signing, administrative acts and their derivatives; - Work with the unified social assistance administration electronic system SOPA and document management system Lietvaris.
