Start date: 14.9.2026 Position as a health and social services worker in a healthcare facility at a Post-Acute Care Center. Requirements: - Education in social work (Dis., Bc., Mgr.) – mandatory - Knowledge of social issues in healthcare settings - Experience in the field is a plus - Compassion for elderly people, pleasant demeanor, empathy, responsibility, independence We offer: - Start date possible from October 2026 - Work on a department at the Post-Acute Care Center - Part-time position (Mon-Fri, 8am-2pm) Job responsibilities: - Patient admissions and discharge coordination - Maintenance of social documentation - Ensuring continuity of care and managing patient social matters - Communication with government agencies and social services - Professional social counseling for patients and their families.
