Dependency Center Stockholm is Sweden's largest addiction clinic. We offer support and assistance to individuals with problems related to alcohol, drugs, medications, or gambling. Our services include acute and 24-hour care, local outpatient care, and highly specialized care. We are also a clinic that provides university healthcare as well as research and education. The Workplace We are now seeking a Care Coordinator for Department 8, located at St Görans Hospital and part of Dependency Center Stockholm's section for acute and 24-hour care. Department 8 receives addiction patients with advanced psychiatric and/or somatic care needs. Our facilities are designed to provide safer, more secure, and more efficient care and work environment, and we maintain the same staffing level every day, around the clock. We care for women separately to increase their sense of safety. Job Responsibilities As a Care Coordinator, you will have a stimulating role that emphasizes individual responsibility while fostering collegial collaboration. You play an important role in the care process and planning for each patient. You conduct individual conversations with patients to investigate their social situation and identify their needs for support and services. The role also involves ensuring that all team members are updated on the information they need to best assist our patients. You will have contact with both internal and external stakeholders, such as social workers, municipal case handlers, residential support providers, treatment facilities, and others. You will participate in network and SIP meetings where an important task is coordinating services and support with municipalities and other partners such as social services and treatment facilities. You are the connecting link between different agencies. Qualifications We are seeking candidates who are trained nursing assistants with professional credentials, licensed nurses, or social workers. For other equivalent qualifications from municipal adult education or older nursing assistant training programs, individual assessment will be made. Experience working as a Care Coordinator and experience with coordinated individual plans (SIP), or alternatively experience as a Case Manager. You must have strong proficiency in Swedish. You have good knowledge of Microsoft Office applications. Meritorious qualifications include prior experience with TakeCare and LifeCare systems as well as experience in addiction care. It is also advantageous if you have experience coordinating between various healthcare and care providers.
