The following documents must be attached to the application for the selection procedure: Signed structured curriculum vitae (capturing previous employment, professional knowledge and skills related to the activities of a civil servant) Certified copy of the document on the highest level of education achieved - the document may be attached to the application as a simple copy Declaration of good conduct pursuant to § 4a paragraph 2 of Act No. 553/1991 Coll. not older than three months Declaration of reliability pursuant to § 4b paragraph 4 of Act No. 553/1991 Coll. not older than three months Extract from the Criminal Record Register not older than 3 months; for foreigners also a corresponding document certifying good conduct issued by the home state not older than 3 months; if such a document is not issued by the home state, good conduct shall be proven by a declaration - documents according to this paragraph may be replaced by a declaration of good conduct when submitting the application An applicant for the position of guard is obliged to notify the person acting on behalf of the municipality in employment law matters (Deputy Mayor of Semily) in writing within 15 days from the date of initiation of criminal prosecution that criminal prosecution has been initiated against him/her. The notification shall be accompanied by a copy of the resolution on the initiation of criminal prosecution or the notification shall state the operative part of this decision, including the designation of the body that issued it. The date of the selection procedure will be specified by email. Submission of application in paper form: Applications may be submitted in person or through the holder of a postal license, in writing in sealed envelopes marked "Selection Procedure - Municipal Police - Do Not Open!" to the address City of Semily, Husova 82, 513 13 Semily. The envelope must include an address to which the application can be returned. The moment of submission is considered to be the moment the recipient receives the shipment. The place for personal submission of the application is the office of the Semily Municipal Office, Riegrovo Square 63. Applications may be submitted on working days - Monday, Wednesday 8:00 - 17:00, on other working days 8:00 - 14:00. The application together with the required attachments must be delivered so that the city of Semily receives it no later than the deadline specified in the announcement of the selection procedure in its data mailbox.
