You lead and plan daily work together with colleagues, primary care centers, and our call center. The work is varied and combines clinical assessments with coordination, patient contact, and development work. You meet patients with different needs and get the opportunity to use your full range of competencies. Examples of tasks Daytime – weekdays Assess referrals and ensure safe medical care Collaborate with primary care centers together with the unit manager and deputy unit manager Responsible for telephone contact with patients and the call center Evenings and weekends Lead and distribute work in the clinical team Conduct planned and acute home visits Contribute to safe and secure care in the patient's home Who are you? We're looking for someone who is confident in your professional role and enjoys working closely with both patients and colleagues.
