Initial contact: by email Job responsibilities: Client communication, needs assessment, connecting clients with specialists and teams, conducting client meetings, organizing client meetings, providing expert counseling (case management), active outreach, service networking, working in a multidisciplinary team (special education specialist, psychologist/therapist), case consultations, team meetings, administrative work related to social work (documentation, preparation, evaluation of client progress), data entry in the e-Quip database, record keeping, reporting, organizational and technical team support. Additional requirements: Communicativeness, responsibility and diligence, ability to work both independently and as part of a team, active management, driver's license (personal vehicle), user-level computer skills, ability for self-reflection and awareness of personal boundaries, clean criminal record.
