Job Duties: Providing field-based personal assistance social services to individuals aged 7 and above without an upper age limit who have reduced self-sufficiency due to health disability, chronic illness, or age and whose situation requires help from another person. This involves individual support for service users in managing everyday life in their natural environment with a focus on maintaining and developing their independence and self-sufficiency. In addition to direct work with users, activities include social assessment and meetings with service inquiries, provision of basic social counseling, contract conclusion, documentation management, individual planning and evaluation of service progress, methodological supervision of staff, service planning and coordination, cooperation with municipalities, institutions, follow-up services and other partners, service promotion and development, participation in meetings, supervisions, training and other professional gatherings. Services are provided throughout the Karlovy Vary Region, particularly in users' homes and other places in their everyday lives.
We Require: Professional qualification for the practice of social work according to Act No. 108/2006 Coll., on Social Services, criminal record clearance, health fitness, Group B driving license, user knowledge of PC work, independence, reliability and responsibility, empathy, patience and respectful approach to people, good communication and organizational skills, ability to work as part of a team, time flexibility and willingness to continue learning and professional development. Previous experience in social work, individual planning and working in the eQuip information system are advantages.
