We are seeking a registered nurse/district nurse for our clinic with an SMO assignment. We are one of four health centers that also has an SMO assignment, Coordinated Medical Care for patients with permanent functional impairments. Kista Health Center has just over 11,300 registered patients and we are located in the middle of Kista Galleria. It takes approximately 17 minutes from the city center by subway with good bus connections. At the health center, doctors, district nurses, registered nurses, nursing assistants, medical secretaries, receptionists, psychologists and a rehabilitation coordinator work together. Work duties: With further education and advanced expertise in diabetes, you will be responsible for receiving and following up patients with diabetes at the clinic. You work with counseling, treatment and continuous follow-up to support patients in their self-care and contribute to safe and secure diabetes care. In primary care you meet patients of different ages and with varying medical conditions at the clinic, both scheduled and acute visits. The assignment also includes typical work tasks such as clinic work, triage, telephone advice, and when needed also home care services. The work requires broad competence and independence. Documentation is done in Take Care and we use the telephone system TeleQ. Type of employment: Permanent position. Full-time. If you have a desire for reduced working hours, this can be discussed. Qualifications: District nurse with prescribing rights and training in diabetes care or licensed registered nurse with specialist training in diabetes care. You must be authorized to practice in Swedish health care. Experience working with diabetes and in primary care is strongly meritorious.
