When illness or functional impairment makes it difficult for patients to visit the health center or hospital, our Home Health Care department is there to provide compassionate healthcare directly in the patient's own home. Health and Medical Care on Gotland is a dynamic department with a rapid development pace that meets patients' needs for care and rehabilitation. The goal of home health care is to provide good and safe care where patients feel secure, involved, and well-received. Operations are organized by geographic areas with multiprofessional teams consisting of district nurses/nurses, nursing assistants, physiotherapists, occupational therapists, and rehabilitation assistants. In total, we have approximately 80 employees distributed across different teams around the island. Since we have several nurses and district nurses within the home health care department, there are excellent opportunities for collaboration, shared and individual development, mentorship, and internal training. We are currently seeking a nurse/district nurse for the Visby team—could that be you? Your responsibilities As a district nurse/nurse in the home health care department, you will carry out planned and sometimes acute interventions within your geographic area of responsibility. The work is independent, and you plan your own days in dialogue with your colleagues. In addition to patient-centered work involving assessments, examinations, information, and advice, you have an important role in collaboration with nursing assistants, occupational therapists, and physiotherapists in your multiprofessional team, as well as with home care staff and primary care physicians. The work also involves frequent contact with other external healthcare providers and patient representatives. Working hours are daytime with duty every fourth weekend. Who are you? We are looking for you—a licensed nurse with professional experience. A B-class driver's license is required. It is advantageous if you have specialist training. You are confident in making independent decisions and assessments, prioritizing based on the situation, and experienced in close collaboration with others. In addition to good teamwork skills, you are relationship-oriented in your interactions with patients and colleagues—it is important that you are perceived as empathetic and responsive. You are a team player who sees the bigger picture, the team's best interests, and keeps the patient in focus. You have strong communication skills in Swedish, both verbally and in writing.
