To Bräcke Home Health Care in Linköping we are seeking licensed nurses who want to work on an hourly basis. Here you will become part of Bräcke Diakoni's combined mission in Linköping City Syd, where care, support, and rehabilitation are located close to each other and collaborate around people who need support at home. Your Role in the Team In home health care you meet people who need care and support in their own homes. Many are elderly or live with disabilities and have contact with several parts of the healthcare and social services system. As a nurse you contribute with both the medical and care perspective and work in a multidisciplinary manner to ensure that interventions work together. You operate from Ramstorps Trygghetsboende together with your nursing colleagues. The team also includes physiotherapists and occupational therapists, with close collaboration with Bräcke's three home care services. You also support care personnel in health and healthcare matters and contribute your assessment when a patient's needs change. You don't need to have worked in home health care before, but you are curious about work where care, support, and rehabilitation meet around the same people. You are confident in making assessments and prioritizing when something changes. You enjoy collaborating with other professions and are comfortable guiding care personnel in health and healthcare matters. Qualifications Licensed nurse Experience with healthcare tasks Class B driver's license Good knowledge of Swedish, both spoken and written Good digital competence and ability to document in medical record systems Meritorious Experience in home health care Experience mentoring personnel in care work Experience in geriatrics or palliative care Experience with the medical record systems Treserva and Cosmic We welcome your application!
