About the position As a care coordinator nurse, you will work as one of four nurses at Eneberg. You are responsible for care coordination for approximately 20 residents, which means you need to have a comprehensive overview of these individuals. Together with the team, consisting of doctors, occupational and physical therapists, nursing assistants, care aides and unit manager, you will make decisions and follow up on implemented measures. Together we develop routines that facilitate the work. Our goal is to work consistently across all our units. Work shifts are currently scheduled during daytime, with work on every sixth weekend. We currently have dedicated nurses working evenings and nights, but some evening shifts may become relevant if conditions change. A driver's license is required for this position, as you will need to be able to travel between the units you are responsible for during weekend shifts. Your work tasks include: Nursing tasks with a focus on care and dementia Guide care staff in various care matters Delegate to care staff Provide telephone advice Work person-centered in areas such as BPSD and palliative care Round work Team meetings About our offer: With us you get independent work with the opportunity to plan your own day together with your closest colleagues. About you For this position you need: Be a licensed nurse Driver's license and driving experience Very good knowledge of Swedish in both speech and writing It is beneficial if you also have: Advanced training in elderly care/palliative care Experience working in nursing homes To thrive with us and fit the role, you need to have a genuine commitment to our elderly and to work in elderly care. We place great emphasis on personal suitability. About us Eneberg is centrally located in Norrtälje and has approximately 70 apartments. We have 9 units, 3 of which focus on dementia care. In October we will convert 7 places to short-term placements for people with dementia. Our goal is to create a home that we ourselves would choose to live in. We are pleased that many of our residents are with us for long periods, which gives us many memorable encounters with our elderly and their loved ones.
