As a registered nurse, you will have a central role in meeting people who have recently been diagnosed with dementia or cognitive illness and their relatives early on. Through early contact, support, and guidance, you create conditions for security, participation, and person-centered care. You provide guidance on the municipality's support and services, strengthen the individual's and relatives' opportunities to receive the right support at the right time, and contribute to preventing the need for more comprehensive interventions. You become an important link between the municipality and the region (health centers and memory clinics) and contribute to good collaboration and coordinated care and support. Duties You will become an important part in creating safe, person-centered, and knowledge-based care for people with dementia and their relatives. You work according to the Health and Medical Services Act (HSL) and contribute with your expertise both in patient care and in the development of the operation. In this role, you will, among other things: Offer conversations and support to patients and relatives Participate in team meetings, care planning, and SIP (coordinated individual plan) Collaborate with different professions and operations Follow research and development within the dementia field and share knowledge Participate in relevant networks for dementia nurses Work person-centered and evidence-based Contribute to the development, implementation, and follow-up of new work methods Contribute to safe care through structured communication and compliance with applicable legislation Qualifications We are looking for you who: Are a registered nurse (180 credits) Have working experience in healthcare Have very good knowledge of Swedish, both spoken and written Have good understanding of relevant legislation and guidelines Have good digital competence Can ride a bicycle It is a merit if you: Have worked with patients with dementia or cognitive illness Have experience of collaboration with health centers and memory clinics We are looking for someone with good collaboration skills who builds trusting relationships by listening, communicating, and finding constructive solutions. You are independent, take initiative, and drive your work forward. You are organized and can plan, prioritize, and organize your work efficiently. You have good oral communication skills and adapt your communication to the person and situation. As a person, you are also creative and innovative and see opportunities to translate ideas into new work methods and concrete results.
