We are a stable health center with just over 8,100 registered patients and offer a range of primary care services – in addition to regular clinics, we also have a Hypertension clinic, an Elderly Care team, and a Women's Health clinic. Read more about the benefits and perks we offer at Närhälsan: Link to our website About the Work Your responsibilities at the health center span a broad range and include general medicine where you meet patients of all ages and in all situations. You work in a health-promoting, advisory, and treatment capacity in first-line care, both by phone, home visits, digital care visits, and at the clinic. The work involves both acute and planned activities. You work largely independently with your own diabetes patient clinic where you triage, treat, and follow up with patients. Close collaboration occurs with your colleagues on the unit and across professions in the team, as well as with other actors such as home care and specialist care. Together with the health center management, you are responsible for developing and advancing the operations. Educating future colleagues is an important and rewarding part of our mission, so mentoring students is also part of your role at the health center. About You You are a licensed nurse with a postgraduate qualification in Diabetes of at least 15 higher education credits. Previous experience as a Diabetes Nurse and/or working in regional primary care is meriting, but we also welcome newly graduated nurses. We seek someone who takes initiative in their work and is comfortable making independent decisions. You are thorough, have good problem-solving abilities, and can effectively prioritize your tasks. You meet many patients in this role, so it's important that you are approachable, attentive, and have a desire to help others. We also believe you enjoy teamwork and have a natural ability to collaborate with colleagues and other contacts in your work. You communicate well both verbally and in writing, and are an engaged and good listener in both digital meetings and clinic visits.
