Job Description Here you will have the opportunity to work as part of an interdisciplinary team providing services to adults within Habilitering's target groups. You will be the only nurse at the clinic where you work closely with physicians and other team members. You will have regular contact with other nurses at our other Habilitering clinics throughout the region. In teamwork, you contribute important pieces based on your professional expertise. Specialized interventions are provided based on health problems related to the patient's functional disability that cause significant difficulties in daily life. Habilitering Borås's catchment area includes municipalities in Södra Älvsborg, and the workplace is located in Borås. The work includes travel to our clinic in Alingsås. Duties Within your area of competence, the nurse's duties are to investigate and assess health problems based on the patient's functional disability, and plan and carry out interventions to give the patient increased opportunities to improve and/or maintain their health. All interventions are planned together with the patient and, if needed, their network. Duties are related to questions about healthy living habits, pain, nutrition, breathing/circulation, skin/tissue, elimination, and pain. Consultation with the patient/network is a recurring intervention. You also work with follow-up according to a national quality register for patients with spina bifida, follow-up of patients with multiple functional disabilities, and patients with neuromuscular diseases. Collaboration with colleagues in regional and municipal primary care and with colleagues in other medical specialties is a natural part of your duties. You frequently coordinate care contacts the patient already has or needs. Interventions can be provided individually or in groups during defined periods. Digital visits are conducted when possible. Work takes place primarily in the facility's premises and, as needed, in the patient's local environment. We are looking for you who - Are a licensed registered nurse with specialist training in open healthcare. It is advantageous if you have several years of professional experience, experience from regional/municipal primary care, and experience working in interdisciplinary teams. - Have experience, interest, and knowledge of working with people in our target group. Further education in psychiatry or equivalent is advantageous. - Are responsible and feel confident working both in a team and independently. - Are flexible, solution-focused, have the ability to structure your work, and have good collaboration skills. - Have a driver's license (category B) as the work involves travel.
