Psychiatric Clinic Nyköping About Us This position is based at the Psychiatric Clinic in Nyköping, which is part of the Division of Psychiatry and Functional Disabilities. The clinic's mission is to provide specialized psychiatric care to adults with severe mental health conditions in southern Sörmland. Our goal is to contribute to improved mental health, increased quality of life, and enhanced functional capacity for our patients. The Psychiatric Clinic in Nyköping is now developing a new initiative for patients in complex life situations who need enhanced and coordinated support alongside the clinic's care services. The work will be based on the evidence-based methods Case Management (CM) and Individual Placement and Support (IPS). Your Responsibilities The Psychiatric Clinic has been granted project funding for two years, with the possibility of an additional one-year extension, to develop a model for enhanced support and coordination in collaboration with, among others, the Swedish Public Employment Service. The aim is to create better conditions for patients to become more independent, strengthen their self-determination, and, based on individual goals and circumstances, move toward work or other meaningful activity. An overarching goal is to develop a sustainable collaboration model that can form the basis for continued regular work after the project period. In your role as coordinator, you will have an important and independent assignment. Together with the patient, you will identify, initiate, coordinate, and follow up on the support measures needed. You will serve as a guide and support for the patient in their contact with various healthcare, government, and support systems, and contribute to creating continuity even during periods when the patient's energy or motivation wanes. The assignment is supportive and coordinating in nature and does not involve treatment responsibility. You will also participate in developing approaches to support patients who wish to move toward or establish themselves in the job market. Collaboration with various actors will be a central part of your daily work. The work is based on the patient's individual goals, needs, and circumstances. The project is led by the clinic's unit manager together with a project manager who has primary operational responsibility for the project's implementation and development. As part of the project organization, a steering group and relevant reference groups will also be established to provide anchoring, guidance, and various perspectives throughout the project. Your Qualifications We are looking for you who are a social worker, occupational therapist, nurse, or have other education and experience that the employer deems appropriate. We are looking for you who want to work directly with patients using evidence-based methods while also being part of developing a new way of working based on patients' perspectives and support needs. You can work independently and in a structured manner, while recognizing the importance of collaboration and being part of the project as a whole.
