We're seeking a new dedicated district nurse who is interested in working in home care; there are also excellent opportunities to shape your position going forward, which can also include work at the clinic itself.
We have a very well-functioning home care service with our own coordinator. You'll have the opportunity to follow patients over time, contribute to good continuity and be an important part of a multiprofessional team.
LidingöDoktorn is a health center with just over 10,900 registered patients. The health center is located in new, fresh premises in Dalénum near the Lidingöbanan station (only 9 minutes from the T-banan at Ropsten).

Job Responsibilities
Medical assessments and follow-ups in the patient's home
Coordination of care services together with doctors, rehabilitation and care staff
Medication management and specimen collection
Care planning and documentation
Patient and family support

Qualifications
We're looking for someone who:
Is a licensed nurse with specialist education as a district nurse
Has experience working in primary care, home care or other relevant outpatient services
Has good ability to work independently and make own decisions
Speaks Swedish well, both verbally and in writing
Has good computer skills and experience with medical record systems
Holds a class B driving license
Meritorious:
Experience in home care
Experience working according to "primary care" choice in Region Stockholm
