JOB RESPONSIBILITIES Are you a confident and committed nurse who wants to work with a holistic perspective and quality in care? We are now seeking a nursing care coordinator who can contribute to patient safety and high-quality care for patients in home health care in regular residential settings. Within Landskrona Omsorg AB there is an HSL group with extended delegations handled by home health care nurses. We are co-located at Kungsgatan 15 together with several professions, which strengthens collaboration and quality for patients in home health care, LSS, and care and residential services. The role includes traditional duties for a nursing care coordinator within municipal primary care, such as: • Assess, plan, implement, and follow up on care interventions for patients with complex needs within municipal primary care. • Collaborate with internal teams and external partners (inpatient care, primary care, ambulance) to ensure a cohesive care process. • Provide guidance, support, and inspire care staff, family members, and other caregivers to promote quality and security in care. • Ensure correct documentation and reporting according to laws, guidelines, and patient safety requirements. • Maintain contact with medical technology and with patients who have complex care needs. You need to have fluent command of Swedish, both spoken and written.
