Key Job Responsibilities: • In collaboration with healthcare professionals, ensure rational use of medications in patient treatment by assessing patient diagnosis, symptoms, and test results, providing evidence-based recommendations on drug therapy, monitoring its effectiveness and safety, and proposing pharmacotherapy adjustments when necessary; • Review medication regimens and participate in the creation and regular revision of the Hospital's drug formulary; • Perform direct medication use monitoring by regularly reviewing test results, interviewing patients, and documenting monitoring data, proposing pharmacotherapy adjustments when necessary; • Conduct medication safety surveillance, including reporting of suspected adverse drug reactions to responsible institutions; • Provide healthcare professionals with consultations on pharmacokinetics, pharmacodynamics, and other matters related to medication prescribing (work in the Drug Information Center); • Participate in the development of Hospital guidelines and recommendations promoting rational medication use; • Participate in multidisciplinary team work according to clinical pharmacist competencies. Requirements for Candidates: • Second-level professional higher education (clinical pharmacist professional qualification); • Knowledge and experience with the regulatory framework governing professional activities; • Precision and high sense of responsibility; • Excellent communication and collaboration skills; • Good command of standard office computer programs; • Good evidence-based information search skills; • Ability to identify and provide recommendations for pharmacotherapy problems (adverse drug reactions, drug interactions, dosing in patients with organ insufficiency or alternative therapy, and others); • Mandatory knowledge of the state language and at least one foreign language (English or German) for working with scientific literature.
