Your Role in the Team
As a physiotherapist with us, you will work with rehabilitation in the patient's home and follow patients over time. You will conduct functional and activity assessments, plan interventions together with the patient, and follow up on how they function in daily life. Your work also includes assistive devices, adaptations to the home environment, and support to care staff in rehabilitation-focused working methods. You will be part of multidisciplinary teamwork where physiotherapists, occupational therapists, and nurses work closely together. We plan and follow up on shared cases, conduct team visits, and support each other as needs change. At the same time, we maintain close collaboration with Bräcke diakoni's home care services, which makes it easier to coordinate services around the patient.

In your interactions with patients, you are responsive and respectful, and you work to ensure the person is involved in their rehabilitation.
Qualifications
Licensed physiotherapist
Experience with independent physiotherapy assessments
Familiarity with documentation in digital health information systems
You express yourself fluently in Swedish, both spoken and written
Category B driver's license
Meritorious
Experience in home healthcare, home rehabilitation, or municipal health and care services
Experience in rehabilitating elderly persons or persons with complex care needs
Experience mentoring care staff
