This role suits you if you appreciate variety and want to work closely with patients, relatives, and other professionals. About the Position You are responsible for planning, implementing, and following up on care based on each patient's individual needs. Your work includes, among other things: Clinical assessments and medication management Planning and monitoring of care interventions Contact with patients and relatives Collaboration and guidance of care personnel Documentation and care planning The position includes evenings and weekends, with an employment level of 75–100%. We Are Looking for You If You Are a licensed nurse Have relevant clinical experience Are comfortable with independent assessments Work in a structured and patient-safe manner Have good collaboration and communication skills Experience from home care, municipal care, VÅBO, or LSS is a plus.
